Insights from The Smile Podcast: A Conversation with Dr. Shivam Pandya

Welcome to another inspiring episode of The Smile Podcast, hosted by Dr. SHAH Smile Studio in Shilaj, Ahmedabad. In this special session, we had the privilege of speaking with Dr. Shivam Pandya, a distinguished surgical oncologist specializing in head and neck cancer surgery. With a wealth of experience and a blog titled Cuts and Bruises, Dr. Pandya has a unique perspective on the challenges and triumphs in the field of oncology.

The Role of a Cancer Surgeon: Restoring Hope

Dr. Pandya eloquently described the role of a cancer surgeon as more than just performing surgeries. “We are in the business of restoring hope,” he said. Many patients come to him feeling hopeless, and the surgeon’s job is to instill emotional and physical healing, often against overwhelming odds.

When asked what inspired him to pursue this field, he took us back to his second year in medical school. A chance to assist in an emergency debridement surgery ignited his passion for surgery. The adrenaline, the problem-solving, and the transformative power of surgery captivated him. However, as he humorously pointed out, being a surgeon requires a “borderline psychopath” mentality to perform such delicate yet invasive procedures.

A Life-Changing Experience

One story stood out: an 18-year-old mentally challenged girl with a stoma. She and her mother traveled from a distant village with no financial resources. Despite the challenges, Dr. Pandya and his team performed life-saving procedures and managed to waive the hospital costs through creative use of hospital policies. When the girl finally spoke her first words to him, “Thank you,” it reinforced the profound impact of their work—not just in saving lives but in providing dignity and hope.

Handling Responsibility and Staying Grounded

Dr. Pandya discussed the immense responsibility doctors face. In the early stages of their careers, some might feel like gods. However, as he experienced setbacks and unforeseen outcomes, he realized that results are often beyond human control. This humbling perspective keeps him grounded and focused on his actions rather than outcomes.

He also shared his “oil-on-water” philosophy, a concept of being empathetically involved with patients while maintaining professional detachment. This balance allows him to provide the best care without being overwhelmed by the emotional toll of the job.

Advances in Cancer Treatment

Addressing the age-old stigma around cancer, Dr. Pandya emphasized the transformative advancements in treatment. From robotic surgery to targeted therapies, patients today have better outcomes and quality of life. He shared inspiring stories of stage 4 cancer survivors who, thanks to multidisciplinary care, live fulfilling lives. The shift in focus is now on rehabilitation and quality of life, ensuring that survivors don’t just live but thrive.

The Role of Rehabilitation

Rehabilitation is a vital part of oncology care. Dr. Pandya collaborated with experts like prosthodontists and physiotherapists to ensure patients regain their confidence and functionality post-surgery. Advanced techniques like CAD-CAM and 3D printing have revolutionized pre-surgical planning and postoperative outcomes, allowing patients to reintegrate into society with dignity.

A Message of Hope

Dr. Pandya left listeners with a powerful message: Cancer is a disease like any other and can often be cured or managed effectively. He urged patients to seek early detection and to engage in open discussions with their oncologists about treatment options. Hope, he reminded us, is the cornerstone of healing.

Closing Thoughts: This episode of The Smile Podcast highlighted the resilience of the human spirit and the incredible advancements in cancer care. Dr. Shivam Pandya’s stories remind us that behind every surgery and every treatment is a mission to restore not just health but also hope and dignity.
